At Freedom Eldercare, we understand how to bridge this gap.  Through a combination of comprehensive resources and proven experience—home health care, geriatric care planning and family education—we provide a seamless and compassionate solution to disease management.

As a licensed, full-service home health care agency, we offer our clients the clinical supervision and accountability necessary for quality disease management. Each client is assessed by a Registered Nurse, who determines a proper care plan. We then arrange specialized home health care, on an hourly or live-in basis and coordinate any other related services needed. Our home health care providers receive intensive disease management training and education, ensuring only the most skilled professionals care for those you love.


Offering Specialized Caregiver Training

Offering specialized in-home caregiver training is unique to Freedom Eldercare. No other company provides the in-depth home health care training we do. At Freedom Eldercare, we do not consider playing videotapes adequate training. Our top notch team of professionals vigorously instructs caregivers on specific diseases. This instruction includes how to best identify and anticipate the special needs and demands of our clients. Our nurses, social workers and Certified Home Health Aides are trained in the latest techniques and have the specialized skills necessary to assist patients with Alzheimer’s disease, Multiple Sclerosis, Parkinson’s disease, cancer, cardiac, and orthopedic conditions.

Teaching with the Hand, Head and Heart

Our professional instructors are passionate about home health care education. We are dedicated to elder care and believe in a compassionate, hands on approach to learning with ongoing monitoring and feedback. We feel this is the best way for our caregivers to learn. This model of teaching and learning ensures that our caregivers are exemplary and compassionate—caring with their hands, heads and hearts.

Our instruction includes a general orientation of State mandated topics. During this time, our team of professionals use a lively, interactive lecture setting to share their specialized knowledge and their passion. The topics are diverse and may include medicine, nursing, social work, customer service, clinical and legal issues.

In addition, we provide all of the New Jersey regulated and mandated curriculum. This includes in-service programs required annually for licensure. In-service programs include courses in infection control, HIPPA compliance, fire safety, and domestic abuse to name a few.

Encouraging Success through Training

We encourage our caregivers to increase their skill levels. We provide additional courses to promote their continued learning, growth and success. Many of our clients often request caregivers with certain skills, such as cooking. In a cooking course students can learn everything from basic nutrition to cooking skills to kosher cooking. Other specialized course subjects include diversity and cultures, advanced clinical skills, and specific disease training.

More than Disease Management

Our approach to disease management goes beyond the care provided by our highly skilled Registered Nurses and professionally trained caregivers. We also provide ongoing social work services. This service model is unique to Freedom Eldercare and enables us to facilitate a cooperative effort on behalf of our clients. Our social workers, in collaboration with our Registered Nurses, monitor and manage all caregiver activity. This includes medical, psychosocial and hands-on care. This ensures coordinated, quality care that includes timely behavioral resolution as well as continuous family feedback and support.

In addition, Freedom Eldercare is one of the few home care companies in the country that employ experienced geriatric care managers. Geriatric care managers offer invaluable guidance and services that often extend beyond medical needs. These services may include legal, financial and social assistance. A geriatric care professional can provide your family with a detailed assessment of care options, crisis intervention or long-term monitoring. They may also facilitate long-distance caregiving, becoming a safety net for families living apart from their aging loved ones.
